GENERAL PROVISIONS

- Enrollment is on a per academic year basis. The student will be considered enrolled for Academic Year .
- All uploaded credentials shall be deemed part of the official school records and can no longer be withdrawn after enrollment
TERMS AND CONDITIONS
SECTION 1. ADMISSION POLICIES

- SJWMS Students
- The following shall be uploaded upon registration:
- a. NSO/PSA Birth Certificate (Required)
- b. Original/Copy Report Card (Form 138) **
- c. Certificate of Good Moral Character from previous school (if applicable) **
- d. Most recent 2X2 ID picture **
- e. Most recent 1X1 ID picture **
- f. Alien Certificate (if foreign national) **
- The student must take the learning assessment test to determine the student's academic strengths. Should the student perform below average, the student shall be accepted but shall be placed under academic probation. The probationary status shall be lifted when the student gets a general average of 78% or higher in any of the first three grading periods provided that he/she passes the other two grading periods. If at the end of the school year, the student remains under academic probation, he/she will be required to take remedial classes during the summer and must obtain a grade of at least 78% to be readmitted for the next school year.
- All documentary requirements must be submitted before the first quarter exam. Students who fail to submit shall not be allowed to take the first quarter exams and risk being dropped from the class roll.
- The student is officially enrolled only when all the documentary requirements have complied with the set period and the enrollment fees are paid.
- The following shall be uploaded upon registration:
- Old Students
- Only students who obtained a passing general average of 75% and have no record of misconduct or misdemeanor shall be readmitted.
- The School reserves the discretion to refuse admission of old students whose parents and/or guardians have placed the School in jeopardy in any manner and/or have unsettled accounts.
- Returning Students
- Students who have transferred to another institution and want to re-enroll to the School are treated as new students in so far as admission policies are concerned.
- SJWMS Students

- The reservation fee of Php 5,000 is non-refundable, non-transferable, but deductible from the enrollment fee.
- Payment of fees may be made on an annual, semi-annual, quarterly or monthly basis. If the enrollment fee is not paid in full, the student shall be considered provisionally enrolled and shall allow attending classes for the first quarter only. All previous payments made shall be forfeited in favor of the School.

- Tuition fees may be subject to discounts such as: early bird, siblings or employee discounts. In case the student is qualified for multiple discounts, the highest discount shall be applied. Application of double discounts is not allowed.
- Miscellaneous fees shall be paid in full regardless of the discount granted.
- Discounts for semi-annual, quarterly, and monthly payment schemes shall be deducted on the last payment period.
- Refer to the Guidelines on SY2023-2024 Discount Offerings.
- Final computation of fees upon discount will be updated in the system within 48 hours.

- Miscellaneous fees are NOT refundable.
- Tuition fees are refundable under the following conditions:

If the withdrawal from enrollment is due to a valid and justifiable reason, the student shall only be charged the proportionate tuition fee up to the last month of attendance. Otherwise, the student shall be charged with the corresponding tuition fee for the entire school year. No credentials and official clearance shall be released unless accounts are settled.
- For the purpose of this provision, “justifiable and valid reasons” shall mean :
- (a) Transfer of residence to another province or country;
- (b) Sickness, duly attested by a medical certificate, that prevents the student from attending classes in any other school for the rest of the academic year; or
- (c) Recommendation of the principal

While the following shall not be considered “justifiable and valid reasons”:
- (a) Voluntary transfer to another school;
- (b) Lack of adjustment to school or to teachers;
- (c) Misunderstandings; or
- (d) Lack of enthusiasm
The School shall have the sole discretion in determination of valid and justifiable reasons. - For the purpose of this provision, “justifiable and valid reasons” shall mean :

- Failure to settle obligations on due dates shall result in a corresponding monthly penalty of ______? until full payment of the amount due is made.
- For students with past due accounts, examinations shall be scheduled on a different date, and grades for a corresponding term shall not be processed until all unpaid fees are settled. The School at its option may suspend the right of the student to receive report cards, academic records, and transcript of records until all unpaid fees are settled.
- The schedule of payment and the amount of penalty shall be issued by the cashier upon admission.

- If the check issued was dishonored by the bank upon presentment for payment/encashment for whatever reason, the School shall notify the parent of such fact. In such event, the said check shall be immediately replaced with its equivalent amount in cash plus a surcharge of PhP 1,200.00 per check and the corresponding penalty if there is a delay in payment (Section 5.1). In addition, the School shall have the right not to accept checks for future payments.
- If the bounced check was issued as payment for official enrollment, such enrollment shall be deemed automatically cancelled

- Transfer Credentials and Student Checklist/Assessment shall not be released to students with unsettled accounts at the end of the school year. A prior clearance shall be obtained from the cashier for the release of any School document.
- Kinder, Grade 6, Grade 10, and Grade 12 students with unsettled accounts are required to pay the balance before the moving up/graduation ceremony. Same policy in Section 7.1 shall apply.

My signature binds me, my spouse, my child and/or the authorized representative/guardian of my child to the terms and conditions as indicated in this Admission Agreement.
