Data Privacy Statement

St. John's Wort Montessori School of Antipolo (SJWMS) recognizes their responsibilities under the Republic Act No. 10173 (Act), also known as the Data Privacy Act of 2012, with respect to the data they collect, record, organize, update, use, consolidate or destruct from the students and alumni. The personal data obtained from this portal is entered and stored within the Institute’s authorized information and communications system and will only be accessed by St. John's Wort Montessori School of Antipolo authorized personnel. St. John's Wort Montessori School of Antipolo have instituted appropriate organizational, technical, and physical security measures to ensure utmost protection of the student/alumnus personal data.

Pre-Registration Process
  • To start off you may click the Pre-Registration Process tab to guide you on how SJWMS Student application works. Feel free to view it anytime.
  • Once you know the online process go ahead and click the Account Registrations tab to fill up the necessary fields for sign-up.
  • You can also view the About SJWMS tab for you to have an overview of details of St. John's Wort Montessori School of Antipolo.

Reminder:

An applicant is only allowed to have one (1) account to apply for one (1) admission evaluation/exam. Multiple accounts, multiple applications, and falsification of submitted information will disqualify your admission to St. John's Wort Montessori School of Antipolo.

Student's Information

       
Student's complete last name based on his/her PSA birth certificate
Student's complete first name based on his/her PSA birth certificate
Student's complete middle name based on his/her PSA birth certificate (Leave it blank if not applicable)

Guardian's Information

Relationship to the Student:*
Guardian's last name based on his/her PSA birth certificate
Guardian's first name based on his/her PSA birth certificate
Guardian's middle name. Leave it blank if none
Guardian's valid and active e-mail address, which you can access anytime. You can only use one (1) e-mail address per online registration in St. John's Wort Montessori School of Antipolo Pre-Registration.
Guardian's valid and active mobile number, which we can communicate you anytime regarding to the student application.

SJWMS Student Application Process

Encoding your student's info for account creation and email validation.
    1.) Go to Account Registration.
    2.) Fill up all the fields with your student and guardian personal informations.
    3.) Then click Create Account.
    4.) Check your email inbox you will recieve an email containing your account logging in instructions.
Once you've recieve an email. You may now continue the pre-registration process in the portal.
    1.) Go to the provided link in the email https://mysj.sjwms.edu.ph/pre_registration/
    2.) Log in your given Reference No. and Password
    3.) Then click Login
    4.) Upon logging in you are required to change the default password provided to you. Just create a new password that you can remember easily and click Update
    5.) It will logout immediately upon changing. Just re-login your new credentials and click Login
  • Step 1: Student Profile
       Fill out all personal and contact information needed.
  • Step 2: Assessment Test
       Take SJWMS's Pre-registration Assessment Test.
    (if this was disabled just proceed to the next step.)
  • Step 3: Student Credentials
       Upload your student required documents.
       Prepare scan copy of the following:
         a) NSO or PSA Birth certificate (required)
         b) Form 138
         c) Certificate of good moral from the previous school
         d) Most recent 2x2 ID picture
         e) Most recent 1x1 ID picture
         f) Alien Certificate (if foreign national)
       Note: JPG, JPEG, PNG & PDF File are the only allowed file
       types to be uploaded.
  • Step 4: Submit Pre-registration Application
       Submission of the basic informations provided.
You will recieve an email containing the link to logon to your student portal account. From that you can now continue your online enrollment in the student portal.
    1.) Go to the provided link in the email https://mysj.sjwms.edu.ph/
    2.) Log in your given Reference No. and Password
    3.) Then click Login
Enrollment is ongoing for the school year 2022 to 2023!
